Overview

CMS Conditions of Participation (CoPs) requires hospitals, including psychiatric hospitals and CAHs, to send electronic patient event notifications of Admission, Discharge, and Transfer (ADT) Event Provider Notifications to another healthcare facility or to another community provider or practitioner. This improves care coordination by allowing a receiving provider or practitioner to reach out to the patient and deliver appropriate follow-up care in a timely manner. Hospitals are required to have a solution in place or risk losing Medicare and Medicaid reimbursement dollars.
