UDS report due

Who we serve · FQHCs and community health centers

Every UDS table should already agree by February 15

One workspace for the whole report, countable visit logic you can defend, and clinical measures calculated with certified eCQM logic. Then close gaps in care during the year so Tables 6B and 7 improve before you report them.

UDS Platform

One workspace, every table

Patients, Visits, Providers, Staffing, Financials, and Clinical Quality Measures in one role based workspace, with countable visit logic validated against the HRSA UDS Manual. Data by flat file, CSV, QRDA Category I, or entered in the interface.

CQMsolution

Measures you can defend

UDS clinical measures calculated with certified eCQM logic and code systems, as HRSA requires, with patient level drilldown. QRDA I from more than one EHR is merged with patient matching and deduplication.

Gaps closed in the visit

Open gaps in care, like an overdue cancer screening or A1c, as cards in the chart while the patient is there, from the same measure logic you report.

FQHC reporting: frequently asked questions

Do we have to replace our EHR to use the UDS Platform?

No. The UDS Platform aggregates data from your EHR and ancillary systems. It accepts flat files, CSV, and QRDA Category I files, and supports data entry in the user interface for anything that does not come through a file.

Do UDS clinical measures follow eCQM specifications?

Yes. HRSA requires UDS quality measures to follow eCQM logic and code systems. The UDS Platform calculates them with the certified eCQM logic in CQMsolution, with checks between countable visits, patient populations, and each measure’s numerator, denominator, and exclusions.

Is UDS+ required for 2026 reporting?

No. UDS+, HRSA’s patient level FHIR submission, is not required for 2026 UDS reporting. Our UDS Reporting Guide covers where it stands, and our breakdown of the 2026 UDS Manual changes walks through this year’s tables.

Do you support rural health centers?

Yes. The Rural Health Transformation Program is putting new state funding into health IT infrastructure. See how DHIT supports rural health transformation.
